Step 2 - Residency Info

Please delete any addresses, phone numbers, and email addresses that are no longer valid. Only newly added addresses, phone numbers, and emails during registration can be edited. If you need to modify an already existing address, phone number, or email address, please delete and re-add it.

The address below must be the address where the student resides.
The phone number should be the number for the primary residence or the parent's mobile number if there is not a landline in the home.
If the student's mobile number is included be sure to select Student Cell as the phone type.
Proofs of residency must be uploaded. Please make sure each document is legible and can be identified.

Each student must establish his/her residency in the following manner:

The parent(s) or legal guardian(s) of a student seeking to enroll must provide the Lamar County School District with at least one of the items from Group I and one of the items from Group II below as verification of their address, except that a document with a post office box as an address will not be accepted.

**Any unofficial Lease Agreement (handwritten or computer-generated) must be notarized.

Group II

Current Utility Bill (dated within thirty days of enrollment or district designated registration)
- Acceptable bills: electricity, gas, water, landline telephone, cable TV or satellite TV.
- Not Acceptable as proof of residency: a driver’s license or voter identification.
Other residency documentation may be approved by LCSD Administration.

Group III

Guardianship
- Student is living with legal guardian and a certified copy of the Court Decree (or petition) if pending, was received declaring the district resident to be the legal guardian of the student and further declaring that the guardianship was formed for a purpose other than establishing residency for school district attendance purposes.

Affidavits

If you are using an affidavit you will need to have the form completed prior to registering. Please visit www.lamarcountyschools.org to download the reqired form. This form must be signed, notarized and uploaded here along with Group I and Group II documents.

Custodial parent(s) residing with an adult in a home not owned or rented by the custodial parent(s) will present documents to show legal custody. The non-custodial adult in the home must prove residency through Group I and Group II (above) and provide a signed, notarized affidavit.

In addition, the custodial parent(s) will present three of the items below:

Insurance policy
Salary check stub
State or Federal benefit check
IRS Documentation
Other residency documentation approved by LCSD administration.

The Lamar County School Board has adopted a policy establishing a Residency Committee within each attendance zone. When a question arises concerning a residency, a parent/guardian will be required to attend a committee hearing to establish residency. Board policy JBC

Step 5 - Immunization & Medical

All students new to the district must provide a record of immunization.

Please add any medical conditions that we should be aware of. This information will be given to the school nurse* and administration. Include as much information as possible under the Notes section when adding a condition.

*Detailed information may be requested by the school nurse after registration.

If your student has any of the conditions listed below we will need additional information for the school nurses.

Asthma/Breathing Problems (Must provide an Asthma Action Plan from a physician. This form can be found on the district website under Departments...Health Care or by contacting the school site nurse.)

Diabetes (A Diabetes Plan/Orders must be completed by a physician. Please contact the school site nurse.)

Epilepsy/Seizures/Convulsions (A Seizure Action Plan must be completed by a physician. This form can be found on the district website under Departments...Health Care or by contacting the school site nurse.)

2025-2026 Chromebook Agreement

Lamar County School District Parent/Guardian Chromebook Responsibility

The Chromebook Check Out Program is a partnership between the Lamar County School District (“LCSD”) and families (“Family”, “Parent(s)”, “Guardian(s)”, “Children” or “Student”). I understand that the LCSD will allow each student the opportunity to check out a Chromebook to use during the school year with the understanding that the family will assume responsibility for the device that is entrusted to them. Damage to the device, while it is in your care, will result in fines which are listed in the Parent Student Handbook under the Technology Acceptable Use Policy. The Chromebooks will be restricted to student accounts. Accounts other than the student account will not be able to log into the device. Chromebooks will have filtered internet access regardless of the connected network. When a Chromebook is issued, the student and parent/guardian are required to indicate that they assume responsibility for the device and any damages that occur to the device while in their care. The Chromebook must be returned to the LCSD immediately upon request by the LCSD

2025-2026 Release of Free/Reduced Information

Asking parents for permission to release Free/Reduced status for counselor use.

I give permission to release my student's Free/Reduced Lunch Status to Counselors and other District Personnel for the purpose of determining eligibility for scholarships, testing fee reductions and other programs.

Step 7 - Primary Contact

This section allows you to add the primary contact for your student. This person will be listed at the top of the contact list and will be the person that is contacted for all school related communications. In the next part of the application you will be able to list all other contacts. Please add as much information as possible for this contact. We must have a current phone number and an email address that you check regularly.

Do not add the primary contact in the next section with the other contacts. If you are a grandparent and you are the primary guardian for your student use Grandparent Custodial as the relationship.

This contact will be the main contact called upon for discipline, attendance and other matters with this child.

AIM Sequence:

If a contact has phone numbers or email addresses set to be used for AIM messages (school-related announcements, emergencies, and other messages), that contact is considered an AIM Contact.

This sequence determines the order in which the AIM Contacts will be contacted when an AIM message is sent out.

The lower the sequence, the earlier the contact will be notified. Contacts with the same sequence will be contacted simultaneously. Contacts with a sequence of N/A will not be contacted.

When an AIM message is sent, the AIM Contacts will be contacted starting with the lowest sequence. Once a contact is reached, the contacts later in the sequence will not be contacted.

Please delete any parents/guardians/contacts that are no longer valid. Please also take care to sequence the contacts carefully. The most important contacts (contacts which the student lives with & emergency contacts) should be at the top of the list.

Please add all relevant contacts on this page. Do not add the person that you listed as the primary contact. If the contact listed has permission to check your student out please indicate that.

If there is someone that should not have access to your student due to a court order please add them in the next section (No Contact Contact) and mark them as No Contact under the Contact Attributes and do not check anything else.
